Guidance on how to Apply for Staff Release from Resorts and Safaris

Following the latest notice issued by Health Protection Agency (HPA) on 10 April 2020 (Notice No.33) regarding the release of staff from tourist facilities, please refer to the attached document on how to apply for staff release from resorts and safaris. There are 3 Options for tourist facilities to choose from.

• Option 1A or 1B - submit Declaration - Exit Form (No.: HPA/DE/2020/004)
• Option 1C - submit Self Quarantine Form (No.: HPA/DE/2020/001)
• Option 2 - submit Declaration - Exit Form (No.: HPA/DE/2020/005
• Option 3 - submit Declaration - Exit Form (No.: HPA/DE/2020/006)

Please note that for Option 1 & Option 2, once the required forms are submitted to Health Protection Agency, ([email protected]) resorts do not have to wait for an approval from HPA.
